DEXFORMS PUBLIC PILOT
A little context
before you begin.
DexForms is open for early testing and feedback. Use it for low-risk forms while we learn from the pilot.
Easy to try. Yours to manage.
You can try the builder without signing in. Creators use ChatGPT sign-in to save and publish. New forms start private; choose Public when publishing to let anyone with the link respond without an account. Publishing as Private again, closing or archiving a form stops new public submissions.
Think about what you ask.
Public form questions are visible to anyone with their link. Avoid asking for passwords, payment details, identity documents or sensitive health information during this pilot. Explain to respondents who you are, why you need their answers and how you will use them.
Answers and photographs are available only in the signed-in owner’s workspace. The owner can export records. The pilot does not yet provide team roles, a complete account deletion workflow or a guaranteed retention schedule; choose your test data accordingly.
Where your information goes.
DexForms stores account details, forms and responses in its hosted database and photographs in file storage. Practice drafts stay in the browser where you created them. Anonymous form sessions last two hours. Short-lived session hashes and hashed network-address counters help limit automated submissions; they do not establish a respondent’s identity.
When email delivery is connected and a creator enables alerts, the email provider receives the creator’s verified sign-in address, a response reference and a workspace link. Answers and photographs are not included. Email delivery is currently awaiting setup.
Know the limits.
The Free pilot includes 100 forms, 1,000 responses per UTC calendar month and 1 GB of photograph storage. Uploads support JPEG and PNG photographs up to 8 MiB each. Usage controls and basic request limits are active. This pilot has no uptime guarantee and is not yet a replacement for a formal compliance or records-management system.
